The Medical Home is more than your local clinic. It is a family centered partnership between your child's personal primary care provider, your child, and you as parents. At the care of a Medical Home is a knowledgeable, compassionate health care provider and care team chosen by a patient and their family to take care of a child's health needs. With 359 days of the year, an on-call physician service 24 hours a day, 7 days a week and additional information available through our website or by phone nurse, we at the Youth Clinic are committed to access, quality and education. The most important aspect of the Medical Home is your relationship with your child's personal care providers or PCPs. They are the best resource for long term care of complex or chronic problems, such as ADHD and other behavioral issues, developmental disabilities, and long term medical issues such as asthma. Your child's personal PCPs coordinates a broad range of community and specialty services as needed for your child and can help to outline long-term treatment goals. They not only know your child's history and needs best, but can work with you to create a secure, trusting relationship in which you and your child can feel safe in dealing with sometimes difficult and painful subjects. For all these reasons, we encourage you to choose two-three personal PCPs for your child who can get to know both your child and you. See them whenever possible, but especially try to make Well Care appointments and appointments for issues that will need long term follow-u-p care with them. We know that your PCPs may not always be available but having two to three providers that you routinely see will help with continuity of care and will heighten your chances in seeing one of your delegated PCPs most times that you schedule an appointment. We welcome you to see any of our providers at any of our three offices for that soccer injury, the earache at daycare, or acute worsening of an ongoing illness. On the weekends, we are very busy seeing acutely ill children, so even if you are able to see one of your PCPs time will be limited for complex problems and focused on the acute illness your child may be experiencing. At the Youth Clinic, to better care for your child and family for those complex and chronic issues we have implemented a Medical Home team. This team is comprised mostly of nursing and business office staff that can assist your family in referrals, in-house and community resources, and assistance programs.
